As an all-volunteer, grassroots organization, WVCDL relies upon our dues-paying members and donors to support our activities. Although WVCDL's officers, directors, and activists volunteer our time, our work is not possible without your financial support.
To join WVCDL, you may download and print one of the membership application forms below. Both files are in PDF format. If you do not have Adobe Acrobat Reader 8 or higher on your computer, you may download and install it by clicking here.
Both versions of the application are the same; however, one version is designed to be printed & completed by hand or typewriter while the other is designed to be typed on your computer and printed, requiring only your signature on the second page:
- Printable application to be completed by hand or typewriter
- Application to be completed by computer & printed (preferred)
Note: If you use the application to be completed by computer & printed, please type your telephone number as 10 digits without any dashes, spaces, etc. (e.g., 3045551212); the application will automatically format the number after you tab to the next field. If you try to type your phone number with dashes or spaces (e.g., 304-555-1212 or (304) 555-1212), you will be unable to enter your phone number correctly.
Individuals who are willing to volunteer to assist WVCDL are encourages to specify in the "Special Skills/Comments" section of the application the types of things you would be willing to do.
